Foundational reports for rehabilitation and case planning

Our Initial Needs Assessments provide a comprehensive overview of the child’s physical presentation, current level of function, and physiotherapy requirements. This is the starting point for our rehabilitation provision for young clients’ who are supported by a case-manager.
INAs help guide the development of therapy goals and ensure a structured approach to care from the outset.

These reports include:

  • A thorough history and understanding of the child, the family and what has led them to seeking physio for their child.
  • A detailed physical assessment including use of a range of measures appropriate to the child and their difficulties.
  • Functional goals and therapy recommendations.

Progress Reports provide a comprehensive review of therapy delivered since the Initial Needs Assessment.

These reports are especially valuable for funding reviews, multi-disciplinary meetings, or Case Manager updates.

These in-depth documents are ideal for scheduled reviews (e.g. 3-, 6- or 12-month intervals) and include:

  • A comparison to baseline/INA findings.
  • A summary of treatment provided to date.
  • Measurable progress against therapy goals.
  • Identification of any barriers to progress (e.g. medical, environmental, emotional).
  • Recommendations for ongoing therapy, adjustments to goals, or additional support.

Update Reports are ideal for case managers and legal teams who require regular updates without full reassessments, or for more complex cases.

Each Update Report may include:
